Information we collect
When you submit an intake we collect the details you provide: eligibility answers (including Medicaid and pregnancy status), names and dates of birth, contact and address information, language preference, food preferences, dietary restrictions, and any notes you choose to share.
How we use it
Only to review your request, contact you, and coordinate food support. We do not sell your information or use it for advertising.
Who can see it
Only authorized, signed-in staff with an assigned role. Access is logged. Applicants cannot view other people's submissions.
Text messages
We only text you if you said yes to text messages. Your consent and its timestamp are recorded, and you can withdraw it at any time by contacting us.
We never include sensitive personal or medical details in email.
Retention
We keep intake records only as long as needed to provide services and meet legal obligations, then delete or de-identify them.
Your choices
You can ask to review, correct, or delete your information by contacting us.
